Vivências de famílias no cuidado à criança e ao adolescente com diabetes mellitus tipo 1: subsídios para a prática de enfermagem

Abstract

O Diabetes Mellitus interfere não só na criança e no adolescente, mas também no seu grupo familiar e social, impondo profundas modificações no estilo de vida. Nesse contexto objetivou-se conhecer as vivências de famílias no cuidado à criança e ao adolescente com Diabetes Mellitus tipo 1. Realizou-se em um Hospital Universitário no Sul do Brasil, no segundo semestre de 2017, uma pesquisa de cunho qualitativo. Participaram 12 familiares cuidadores de crianças e adolescentes atendidas no Centro Integrado de Diabetes. Os dados foram coletados por entrevistas e submetidos à Análise de Conteúdo. Foram respeitados os princípios éticos da pesquisa envolvendo seres humanos. Logo o projeto possui aprovação do Comitê de Ética em Pesquisa, CAAE: 68873117000005324, parecer nº 114/ 2017 Verificou-se que é comum a criança internar na urgência, receber o diagnóstico ao consultar por outro motivo ou por desconfiança da família acerca dos sinais e sintomas apresentados. Após o diagnóstico a família busca informações que a subsidie ao cuidado, referiram como positivo serem informados acerca da doença, poderem esclarecer dúvidas, aprenderem a técnica de administração da insulina e acerca da dieta a ser utilizada. Além do profissional de saúde, a família conta com auxílio de serviços especializados em Diabetes. Acreditam como importante a convivência da criança e do adolescente com outros com Diabetes, aprendem a administrar a insulina e preocupam-se em garantir seu acesso rápido aos serviços de saúde. Referiram como dificuldades para o cuidado o fato da mesma ser diferente das outras crianças e adolescentes, a não aceitação da patologia e não adesão ao tratamento, dificuldades financeiras e a sobrecarga do cuidador principal. Referiram preocupação da possibilidade da criança e do adolescente apresentar complicações da doença. A principal fonte de apoio da mãe para o cuidado é a família nuclear, sendo o marido a principal figura de apoio. Concluiu-se que o reconhecimento da família como sujeito ativo no cuidado é importante para o planejamento das ações de cuidado e apoio. Acredita-se que este estudo possibilitou a construção de conhecimentos que podem contribuir para qualificar o fazer do enfermeiro no atendimento a essas famílias e à criança e ao adolescente acometida de Diabetes Mellitus tipo 1.

Diabetes Mellitus interferes not only with the child and adolescent, but also with their family and social groups, imposing profound changes in lifestyle. In this context, we aimed to know the experiences of families in the care of children and adolescents with Type 1 Diabetes Mellitus. A qualitative research was carried out in a University Hospital in the South of Brazil in the second half of 2017. Twelve family caregivers of children and adolescents attended at the Integrated Diabetes Center participated. Data were collected through interviews and submitted to Content Analysis. The ethical principles of research involving human beings have been respected. The project was approved by the Research Ethics Committee, CAAE: 68873117000005324, opinion no. 114/2017 It was verified that it is common for the child to go into the emergency room, to receive the diagnosis when consulting for another reason or for the family's distrust of the signs and symptoms. After the diagnosis, the family sought information that subsidized care, referred as positive to be informed about the disease, to clarify doubts, to learn the technique of insulin administration and about the diet to be used. In addition to the health professional, the family counts on the assistance of specialized services in Diabetes. They believe that it is important for the child and adolescent to coexist with others with Diabetes, they learn how to administer insulin, and they care about ensuring their rapid access to health services. They referred as difficulties to care the fact that it is different from other children and adolescents, not accepting the pathology and not adherence to treatment, financial difficulties and the overload of the main caregiver. They reported concern about the possibility of the child and adolescent presenting complications of the disease. The mother's main source of support for care is the nuclear family, the husband being the primary supportive figure. It was concluded that the recognition of the family as an active subject in care is important for the planning of care and support actions. It is believed that this study allowed the construction of knowledge that can contribute to qualify the nurse's role in the care of these families and the child and adolescent affected with Type 1 Diabetes Mellitus.
